COMPLETE THE ABOVE LIFE SETTLEMENT APPLICATION which includes the following:
• Application for Life Settlement
• HIPAA Authorization (Medical Information Release)
• Authorization for the Release of Policy Information
Provide us with the following illustrations:
Universal Life:
An in-force illustration that show a level death benefit with minimum level annual premium payments and cash value as close to zero as possible at age 100.
Term Life:
The current term schedule as well as an illustration that shows a conversion to Universal Life.
Whole Life:
An in-force illustration surrendering the dividend and paid up additions (PUA) option to reduce ongoing premium with a level death benefit. If there is an existing loan on the policy, pay the loan off in the 1st year of the illustration.
Also, if available please provide all medical records. Medical Records submitted should include all documents pertaining to the insured and should include at least the two year period immediately preceding the date upon which the case is submitted. The term “medical records” is defined to include any and all documents produced by each and every healthcare provider or facility of any kind which has, over the period of time in question, produced any medical analysis, information, test results or other documentation pertaining to the covered insured.
